Who is HACC for?
The program supports people living in Tasmania who are:

Aged under 65

Aboriginal and Torres Strait Islander people aged under 50.
Eligibility is based on an assessment of needs and takes into account a difficulty you have in doing daily tasks.
Who can refer the TCCRS?
Anyone can contact TCCRS to refer someone for service including:

other service providers.
How can I be considered for the HACC program?

The Tasmanian Community Care Referral Service
(TCCRS) provides easy access to information and referrals for Tasmanian Government-funded Tasmanian HACC services through a single phone call. It can help you:

understand which services are needed

assist you in gaining access to them.
How the referral process works
Step one
They will collect some basic information to assess your eligibility and the services you may need.
Step two
TCCRS will then refer you to the most appropriate service provider in your area.
Step three
That service provider will contact you.
